You have had a difficult day, and by evening your shoulders feel unusually tight. Your jaw has been clenched without you realizing it, your neck feels stiff, and there is a heaviness in your body that seems disproportionate to what you have physically done. At the same time, you are mentally exhausted. Perhaps you have been worrying about a decision, replaying an uncomfortable conversation, managing several responsibilities at once, or simply functioning under more pressure than usual. At this point, it can become surprisingly difficult to answer a basic question: Is what you are experiencing emotional tension or physical tension? In everyday life, the two are rarely as separate as those labels suggest. Physical tension can certainly arise from posture, repetitive movement, exercise, injury, prolonged sitting, sleep position, or other mechanical and medical causes, but psychological stress can also influence muscle activity, breathing, pain perception, sleep, and the way we unconsciously hold the body. Research has explored relationships between mental stress, sustained muscular activity, and musculoskeletal discomfort, particularly in the neck and shoulders (Lundberg et al., 1994; Westgaard, 1999).
This overlap helps explain why body-based practices such as acupressure are often used for both physical discomfort and emotional stress. Acupressure involves applying pressure to particular areas of the body and has been investigated for concerns ranging from pain to anxiety. Some systematic reviews have found encouraging results, although the quality, consistency, and strength of the evidence vary considerably depending on the condition being studied (Au et al., 2015; Chen et al., 2022). It would therefore be inaccurate to describe acupressure as a technique that simply removes “stored tension” from the body or treats every physical and emotional symptom through the same mechanism. A more useful approach is to understand how emotional and physical tension interact, what acupressure might realistically contribute, and how a physical practice can sometimes influence emotional experience without requiring us to assume that emotions are literally stored inside particular muscles or pressure points.

Emotional Tension and Physical Tension Are Different, but They Are Not Separate Systems
Emotional tension generally describes a state of psychological strain. You might feel worried, irritable, restless, apprehensive, frustrated, pressured, or unable to mentally settle. Physical tension, by contrast, is usually experienced through the body: muscles feel tight, the shoulders become stiff, the jaw clenches, the neck aches, or certain areas feel as though they cannot fully relax. These definitions are useful for describing experience, but biologically the distinction is less tidy because emotional states and bodily processes continually influence each other. Stress can change breathing, posture, movement, sleep, attention, and muscular activity, while physical pain or discomfort can influence mood, concentration, patience, and perceived stress.
Imagine that you have an important meeting in the afternoon. You spend the morning mentally rehearsing what you are going to say, anticipating difficult questions, and worrying about how other people might respond. You are not doing strenuous physical activity, yet without realizing it you sit more rigidly than usual, tighten your jaw, elevate your shoulders, and remain relatively still for long periods. By the time the meeting is over, your neck aches. The discomfort is physically real, even though psychological stress may have contributed to the conditions under which it developed. Experimental research has shown that mental stress can influence muscle activity, including during relatively low physical workloads, which provides one possible pathway through which psychological demands may contribute to musculoskeletal discomfort (Lundberg et al., 1994).
At the same time, it would be a mistake to conclude that every tight shoulder or aching neck is caused by stress. Physical symptoms can arise from repetitive strain, exercise, injury, arthritis, nerve irritation, ergonomics, sleep position, illness, and numerous other causes. Even the assumed relationship between anxiety and muscle tension is more complicated than popular descriptions sometimes imply. A critical review by Pluess et al. (2009), for example, found that the relationship between generalized anxiety disorder and objectively measured muscle tension was not as straightforward as conventional assumptions might suggest. Understanding the mind-body connection therefore requires nuance. Psychological stress can influence physical experience, but recognizing that connection should never become a reason to dismiss physical symptoms as “just emotional.”
Emotional Tension May Show Up in the Body Before You Recognize It as Stress
Not everyone experiences stress primarily as conscious worry. Some people know immediately when they are anxious or emotionally overloaded, while others notice physical changes first. They may realize that they have been clenching their teeth, repeatedly rubbing their neck, tightening their abdomen, holding their breath while reading messages, or sitting with their hands tightly folded. They may describe themselves as coping perfectly well until a headache, aching shoulders, exhaustion, or difficulty sleeping makes it obvious that the week has taken a toll.
Consider someone managing an intense period at work while simultaneously supporting a family member through a difficult situation. If you ask how they are doing, they might genuinely answer, “I'm fine. I'm just busy.” Their attention is focused on functioning: completing tasks, responding to people, remembering appointments, and solving problems. There may be little time to examine their emotional state. Yet by the end of the week, their shoulders are persistently tight, their sleep is poor, and their body feels unable to settle even when nothing urgent is happening. When they finally stop moving, they realize how much internal pressure they have been carrying.
This does not mean their shoulders have literally “stored” the week's stress. Language such as “stress is stored in your shoulders” or “anger lives in your jaw” can be emotionally intuitive, but it should not be confused with established physiology. A more evidence-based interpretation is that prolonged psychological demand can influence muscle activation, posture, movement, breathing, sleep, and pain sensitivity. If someone repeatedly tightens the same muscles while stressed, the association between a psychological situation and a physical pattern can become personally meaningful without proving that a particular emotion resides inside that muscle.
Your individual patterns can still provide useful information. If you consistently notice that your jaw tightens during conflict, recognizing that response may help you notice emotional activation earlier. If your shoulders become rigid every time you open your work email, that pattern may tell you something about how your body responds to anticipation. The goal is not to assign universal emotional meanings to body parts but to become more attentive to the relationship between your own circumstances and physical experience.
Physical Tension Can Influence Your Emotional State Too
The mind-body relationship works in both directions. Persistent physical discomfort can make emotional regulation more difficult because pain and tension consume attention, interfere with sleep, restrict movement, and increase the effort required for ordinary activities. Someone who has been uncomfortable for several hours may become less patient, more mentally fatigued, or more sensitive to additional demands. This does not mean their emotions are caused entirely by their muscles; it means that emotional experience occurs within a body whose current condition influences how much capacity is available.
Imagine waking after a poor night's sleep with significant neck stiffness. Your morning commute is uncomfortable, working at a computer makes the pain worse, and concentrating requires more effort than usual. By afternoon, a colleague asks a relatively ordinary question and you feel disproportionately irritated. On another day, the same question would barely register. Today, however, you are already using psychological resources to tolerate discomfort. The physical tension has changed the context in which emotional regulation is happening.
This is particularly relevant when discussing whether acupressure can help with “both” physical and emotional tension. If a physical intervention reduces discomfort, emotional well-being may improve indirectly. Feeling less pain may make it easier to sleep, concentrate, move, communicate, or tolerate frustration. Similarly, if someone becomes less anxious, they may naturally unclench their jaw, lower their shoulders, move more freely, or breathe differently. An improvement in one domain can influence the other without demonstrating that exactly the same biological mechanism caused both changes.
What Is Acupressure, and Where Might It Fit?
Acupressure developed within traditions associated with acupuncture and traditional Chinese medicine. Rather than inserting thin needles into the skin, acupressure typically involves applying manual pressure to specific points using the fingers, hands, or sometimes specialized devices. Within traditional Chinese medicine, these points are understood in relation to meridians and the movement of qi. Contemporary biomedical research generally approaches acupressure differently, evaluating outcomes such as pain, anxiety, nausea, fatigue, and sleep while exploring possible physiological and psychological mechanisms.
It is important to keep these explanatory systems distinct. Traditional concepts are part of the historical framework from which acupressure developed, but claims that pressing one point directly “releases stored trauma,” removes a specific emotion, or instantly resets the nervous system are stronger than current scientific evidence supports. Even acupuncture, which has been researched considerably more extensively than acupressure, does not have one universally established mechanism explaining all of its reported effects. The National Center for Complementary and Integrative Health (NCCIH) notes that acupuncture may influence nervous-system function and local tissues while also recognizing that nonspecific effects associated with context, expectations, and the treatment experience may contribute to outcomes (NCCIH, 2024).
A scientifically cautious way to understand acupressure is therefore as a complementary sensory practice that may influence certain physical or psychological symptoms for some people. That definition may sound less dramatic than claims about releasing energy or unlocking emotions, but it gives us a much stronger foundation for examining whether acupressure might be useful when physical and emotional tension occur together.
What Does Research Say About Acupressure and Emotional Tension?
Researchers do not generally study “emotional tension” as one clearly defined clinical outcome, so anxiety provides one of the closest bodies of evidence. A systematic review and meta-analysis by Chen et al. (2022) examined 27 randomized controlled trials evaluating acupressure for anxiety. Overall, the researchers found reductions in anxiety associated with acupressure. However, the studies differed substantially in the populations being treated, the points used, the duration and frequency of treatment, comparison conditions, and methodological quality. The authors therefore emphasized that the high level of heterogeneity required cautious interpretation.
An earlier systematic review and meta-analysis by Au et al. (2015) reached a similarly measured conclusion. Acupressure appeared to provide immediate relief from anxiety associated with medical treatment or procedures among adults, although physiological outcomes were less consistent and weaknesses in study reporting limited the certainty of the conclusions. More recent research has continued to investigate acupressure in situations involving short-term anxiety. Xie et al. (2023), for example, conducted a systematic review and meta-analysis involving adults undergoing elective surgery and found that acupressure was associated with reduced preoperative anxiety.
These findings are promising, but their meaning should not be stretched beyond the populations and outcomes actually studied. Reduced preoperative anxiety does not prove that acupressure treats chronic stress, trauma, generalized anxiety, relationship distress, or every form of emotional tension. What the evidence more reasonably suggests is that acupressure may help reduce subjective anxiety or immediate psychological tension in some circumstances. For someone using acupressure during an emotionally difficult day, that limited effect may still be valuable. A complementary practice does not have to resolve the cause of distress to provide a useful moment of relief.
What Does Research Say About Acupressure and Physical Tension or Pain?
The evidence becomes more complicated when we turn to physical tension because “tight muscles” is not one medical condition. Neck stiffness after eight hours at a computer is different from chronic low back pain, an acute injury, arthritis, neuropathic pain, or muscle soreness following exercise. Research on acupressure therefore tends to focus on particular pain conditions rather than physical tension in general.
Systematic reviews have found encouraging results in several areas. Research on acupressure for low back pain, for example, has reported reductions in pain in some randomized trials and reviews, although differences in interventions and study quality remain important limitations. Acupressure and related approaches have also been investigated for labor pain and cancer-related pain. He et al. (2020), in a systematic review and meta-analysis published in JAMA Oncology, found that acupuncture and acupressure were associated with reductions in cancer pain and analgesic use, although the authors emphasized the need for additional rigorous trials.
The broader evidence on acupuncture also suggests that stimulation of particular points may influence some pain conditions, although acupuncture and acupressure should not be treated as interchangeable simply because they share historical concepts. NCCIH reports evidence supporting acupuncture for several pain conditions while emphasizing that effects differ across conditions and that researchers continue to investigate the relative contributions of physiological and nonspecific mechanisms (NCCIH, 2024).
For someone experiencing ordinary muscular tension, the practical implication is modest. Gentle acupressure may feel soothing and may reduce discomfort for some people, but persistent or significant physical symptoms should not automatically be treated as stress-related muscle tension. Pain that is severe, worsening, follows an injury, repeatedly returns, or occurs alongside symptoms such as weakness, numbness, swelling, fever, chest pain, or other concerning changes deserves appropriate medical evaluation.
Can Acupressure Affect Physical and Emotional Tension at the Same Time?
Suppose you have just finished an uncomfortable conversation. You feel emotionally agitated, your shoulders are raised, your jaw is tight, and your thoughts keep returning to what was said. You sit somewhere quiet and apply comfortable pressure to an appropriate acupressure point for several minutes. While doing so, you become aware of how rigidly you are sitting. Your shoulders gradually soften, and by the end of the pause you also feel somewhat less emotionally activated. Has acupressure treated two separate forms of tension simultaneously?
Possibly, but the explanation is unlikely to be that one pressure point simply switched off both conditions. Several processes may have occurred at once. Sitting quietly gave your muscles an opportunity to stop working as hard. Focused sensory attention interrupted repetitive thinking. Your breathing may have changed naturally. Time passed, allowing the initial emotional intensity of the conversation to decrease. The pressure itself may have influenced your perception of discomfort, while the expectation of relief or the experience of intentionally caring for yourself may also have contributed.
None of these possibilities makes the experience less useful. In fact, they illustrate something important about mind-body practices: their effects may emerge from several interacting processes rather than one dramatic mechanism. Because physical sensation, attention, emotion, pain perception, expectation, and behavior are interconnected, a practice that changes one part of the experience can sometimes influence the others.
Instead of asking whether acupressure “removed” emotional and physical tension, a more useful question may be: Did this practice help me become physically more comfortable and emotionally more capable of responding to what is happening? That outcome is meaningful without requiring us to make claims that the evidence cannot support.
Why Attention May Be Part of the Effect
One of the simplest explanations for why acupressure may feel regulating is that it changes what you are paying attention to. Emotional tension often involves repetitive mental activity. You replay what someone said, anticipate what might happen tomorrow, mentally defend yourself in a conversation that has already ended, worry about unfinished responsibilities, or attempt to solve several problems simultaneously. When attention becomes tightly captured by this stream of thought, emotional activation can feel increasingly intense.
Acupressure introduces a competing sensory experience. You notice your thumb pressing against your wrist, warmth between your fingers and skin, the position of your arm, the texture beneath your hand, or changes in muscular tension as you sit. The stressful thought has not disappeared, but it is no longer the only information entering conscious awareness. This kind of sensory attention has some conceptual overlap with grounding practices, which are used to help people reconnect with immediate experience during periods of stress.
The body's internal signals are also relevant. The perception and interpretation of bodily states is known as interoception, and research increasingly recognizes its relationship with emotional experience and regulation. Khalsa et al. (2018) described interoception as a central process relevant to mental health, while Price and Hooven (2018) proposed that mindful interoceptive awareness can contribute to emotional regulation by helping people identify and respond to bodily cues more effectively. This does not mean that greater attention to the body is always calming, but it provides a useful framework for understanding why deliberately noticing physical sensations may sometimes change emotional experience.
During acupressure, then, the psychologically useful part of the practice may extend beyond the specific point being pressed. The pause itself may help you notice information that was being overlooked. Perhaps you realize that your “anxiety” is partly exhaustion. Perhaps you notice that your neck tension increases primarily after long periods at the computer rather than during emotionally stressful situations. Or perhaps you recognize that your shoulders consistently tighten when a particular topic arises. These observations help you understand patterns without forcing you to classify the experience as either purely emotional or purely physical.
Physical Relief Does Not Necessarily Resolve Emotional Stress
Suppose acupressure reduces the discomfort in your shoulders after a difficult workday. That relief is valuable, but it does not necessarily mean that the circumstances producing your emotional tension have changed. If the tension developed while dealing with an unreasonable workload, you may still need to reconsider deadlines, ask for support, or establish limits. If your jaw becomes tight after repeated conflict with someone close to you, relaxing the jaw does not resolve the relationship problem. If your body feels constantly braced because you are anticipating criticism at work, symptom relief may help you cope without addressing why you feel that you need to remain braced in the first place.
This is not an argument against symptom relief. Temporary relief can be useful and humane. We routinely use supportive strategies while larger problems are being addressed. The difficulty arises when regulation becomes a way of repeatedly adapting ourselves to circumstances that require change. A person can become very skilled at calming themselves after every stressful interaction while never asking why those interactions are happening so frequently.
A more constructive sequence is to use regulation to reduce immediate intensity and then become curious about what remains. Once your body feels somewhat more settled, ask whether anything actually requires action. Perhaps nothing does. Maybe you simply had a demanding day and needed recovery. But perhaps you recognize that a boundary has been crossed, your workload is unsustainable, you have not been sleeping enough, or a relationship is repeatedly leaving you tense and emotionally depleted. In that case, acupressure may have helped not by solving the problem but by creating enough space to see it more clearly.
Emotional Relief Does Not Mean Physical Symptoms Should Be Ignored
The opposite mistake can also occur. Suppose you notice that your neck feels better whenever your stress decreases. It might be tempting to conclude that the pain is purely psychological and therefore does not need further evaluation. Yet physical symptoms can have multiple contributing factors. Stress might aggravate an existing musculoskeletal problem without being its original cause, or a physical condition might become more noticeable during periods of stress because sleep and recovery are worse.
This is why mind-body explanations should expand our understanding rather than replace medical reasoning. A symptom can be influenced by psychological stress and still deserve physical assessment. Likewise, a person can have a genuine musculoskeletal condition and experience additional tension when anxious. The two possibilities are not mutually exclusive.
If discomfort is persistent, severe, worsening, recurrent, associated with an injury, or accompanied by neurological or systemic symptoms, professional assessment is appropriate. Similarly, if emotional tension is persistent enough to interfere with sleep, relationships, concentration, work, or everyday functioning, it deserves attention beyond a relaxation practice. Acupressure may fit alongside appropriate care, but it should not become a reason to postpone understanding what is actually happening.
Using Acupressure as a Mind-Body Check-In
One of the most practical ways to use acupressure when you are unsure whether your tension is physical, emotional, or both is to treat the practice as a short period of observation rather than an attempt to diagnose yourself. Sit comfortably and apply gentle, tolerable pressure to a suitable acupressure point that you have learned to use safely. Instead of immediately trying to make the tension disappear, notice what is happening. Where is the physical discomfort located? Does it feel like tightness, aching, pressure, fatigue, or something else? Does your posture change when you think about a particular situation? Are you holding your breath, clenching your jaw, or lifting your shoulders without realizing it?
Then widen the question slightly. Ask yourself what was happening before you noticed the tension. Perhaps you have been sitting for three hours without moving, which points toward an obvious physical contribution. Perhaps the tension appeared immediately after opening a particular email. Perhaps both are true: prolonged computer work has physically strained your neck, while anxiety about work has increased muscular bracing and made the discomfort more noticeable.
This type of observation avoids the need to decide that a symptom belongs exclusively to the mind or body. Instead, you begin identifying contributing factors. Sometimes the appropriate next step is movement, rest, hydration, sleep, an ergonomic adjustment, or medical assessment. Sometimes it is a difficult conversation, a boundary, emotional support, or a change in workload. Sometimes the best response includes both physical and psychological care.
The purpose of the acupressure pause is therefore not simply to ask, Did the tension disappear? It can also help you ask, What might this tension need from me?
When Body Awareness Is Not Relaxing
Although focused physical attention can be calming for many people, it is important not to assume that turning inward always feels safe. Some people become more anxious when they concentrate on bodily sensations, particularly if they experience panic, health anxiety, trauma-related symptoms, or dissociation. A racing heartbeat, tight chest, or altered breathing may become more frightening when attention is focused intensely on it.
Interoception research helps explain why bodily awareness is not automatically beneficial. Emotional regulation depends not simply on detecting internal sensations but also on how those sensations are interpreted and whether the person can attend to them without becoming overwhelmed. For one person, noticing a faster heartbeat may provide useful information that they are becoming anxious. For another, the same awareness may trigger the fear that something medically dangerous is happening, which further increases anxiety.
If focusing on acupressure or internal sensations makes you feel increasingly frightened, trapped, detached, or overwhelmed, there is no reason to force yourself to continue. External grounding may be more appropriate. You can look around the room, notice colors and objects, listen to sounds, feel your feet against the floor, walk gently, or speak with someone you trust. A regulation practice should increase your ability to remain present rather than become another experience you believe you must endure.
Sometimes Tension Is a Message About Your Life, Not Just Your Muscles
Perhaps the most important question is not always how to reduce tension but why you keep needing to reduce it. If your shoulders become painfully tight every evening because you spend ten hours at an unsuitable workstation, the longer-term solution may involve ergonomics and movement rather than finding a stronger acupressure technique. If you need to calm yourself after nearly every conversation with a particular person, there may be a relational problem that deserves attention. If your body remains tense because you are chronically sleep-deprived, relaxation techniques cannot substitute for adequate rest.
Repeated tension can therefore become information. It may tell you that your physical environment needs adjustment, that you need more recovery, that your workload has exceeded your capacity, or that an emotional situation remains unresolved. Acupressure may help you hear that information because it interrupts automatic momentum for a few minutes. But the practice becomes more useful when the pause leads to appropriate action rather than becoming an endless cycle of tension, temporary relief, and renewed tension.
There will also be times when tension is simply part of being human. A stressful week can make your shoulders tight without indicating a profound psychological problem. Exercise can make muscles sore. An uncomfortable conversation can leave you physically activated for a while. Not every sensation requires interpretation or treatment. The aim is not to become hypervigilant about the body but to develop enough awareness to recognize patterns when they matter.
Conclusion: Physical and Emotional Tension Often Meet in the Same Body
Emotional tension and physical tension are not identical, but they do not occur in completely separate systems. Psychological stress can influence muscle activity, posture, breathing, sleep, attention, and the experience of pain, while physical discomfort can affect mood, concentration, patience, and emotional capacity. The relationship is interactive, individual, and often more complicated than simple statements such as “stress causes tight muscles” suggest.
Acupressure may have a useful place within this interaction. Research suggests potential benefits for anxiety and certain pain conditions, although evidence varies across populations and studies, and acupressure should not be presented as a universal treatment for either emotional or physical tension (Au et al., 2015; Chen et al., 2022). Its value may also come partly from something more ordinary: it asks you to stop for a moment, direct attention toward physical sensation, and notice what is happening rather than continuing automatically through stress.
Sometimes that pause may soften muscular discomfort. Sometimes it may reduce subjective anxiety. Sometimes it may simply reveal that you are exhausted, sitting badly, worried about something, or carrying more responsibility than you currently have capacity for. Those observations can be useful even when the pressure itself does not make the tension disappear.
The goal does not have to be deciding whether tension is “in your mind” or “in your body.” A more helpful question is what factors may be contributing to the experience and what kind of response they require. Perhaps your muscles need movement and recovery. Perhaps your emotional world needs attention. Perhaps a practical situation needs to change. Often, several things are true at once.
Acupressure can be one way of creating enough quiet to notice the difference. It may help with aspects of both physical and emotional tension, but its most useful role may be as part of a larger habit of listening: noticing what the body is communicating, considering what is happening psychologically, and responding to both with curiosity rather than assuming that one must be the cause of the other.
